How our plan works
No ID cards or pre-authorization required. Simply select a PVCS network provider and identify yourself as a PVCS member when scheduling your appointment. The provider will be able to verify eligibility and benefits using your date of birth and member id number.

Routine comprehensive eye exams are covered 100% and not limited to once a year. All materials at wholesale cost. (Frames, lenses, lens options and contact lenses) No limit on prescription glasses and annual supply of contact lenses.
